Our Commitment

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Pure Hormone Health LLC is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI.

How We May Use and Disclose Your Health Information

• Treatment — to provide, coordinate, and manage your telehealth care, including sharing prescriptions with a licensed compounding or retail pharmacy and orders with a laboratory.
• Payment — to obtain payment for services, including processing your private-pay membership or visit fees.
• Health Care Operations — for quality review, clinical documentation, credentialing, and administration of the practice.
• Business Associates — with vendors who perform services on our behalf (scheduling, electronic records, secure video, billing) under a written HIPAA business associate agreement.
• As Required by Law — including public health reporting, health oversight activities, judicial or administrative proceedings, law enforcement requests, and to avert a serious threat to health or safety.

Uses That Require Your Written Authorization

Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of protected health information require your written authorization. We do not sell your health information. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

Your Rights Regarding Your Health Information

• Inspect and copy your records, including an electronic copy of records we maintain electronically.
• Request an amendment if you believe information in your record is incorrect or incomplete.
• Request an accounting of certain disclosures we have made of your health information.
• Request restrictions on certain uses and disclosures. We must agree to restrict disclosure to a health plan when you pay in full out of pocket for a service.
• Request confidential communications by an alternate means or at an alternate location.
• Receive a paper or electronic copy of this notice on request.
• Be notified if a breach occurs that compromises the privacy or security of your information.

Telehealth and Website Information

Clinical care is delivered by secure, HIPAA-compliant video and documented in our secure patient platform. The general inquiry form on this website is not a secure clinical channel. Please do not submit diagnoses, symptoms, medications, lab results, or insurance identifiers through the website form.

How to Exercise Your Rights or File a Complaint

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington, D.C. 20201, or at hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future. The current notice will always be posted on this page with its effective date.
