Membership, Billing & Cancellation

Nature of Fees

Pure Hormone Health LLC is a private-pay telehealth practice. We do not bill insurance for membership or visit fees. Laboratory testing is billed separately by the laboratory or is an out-of-pocket cost, as explained before you order. Compounded hormone costs depend on the care track you choose.

Discovery Call

The 15-minute discovery call is complimentary. It is used to confirm that we are licensed in your state and that the practice is an appropriate fit. It is not a medical visit and no prescription is issued on that call.

Concierge Membership

Women's Concierge: $300 initial visit, then $250 per month. Men's Concierge: $250 initial visit, then $200 per month. The monthly fee includes clinician access by televisit and the compounded hormone prescription that is part of that track (Brite organic-oil formulations). It does not include 24-hour urine testing, extra laboratories, or medications that are not part of the agreed protocol.

Essential Track

Essential care is $150 for the initial visit plus the cost of cream-base compounded hormones and laboratories. A telehealth follow-up is required at least every three months and is $50 per visit.

How to Cancel

You may cancel a Concierge membership by written notice to the practice (email or patient portal) at least five business days before the next billing date. Access continues through the period already paid. We do not prorate a month that has already begun unless required by law.

What Is Not Refundable

Completed visits are not refundable. Compounded prescriptions that have already been prepared or shipped cannot be returned or refunded. Laboratory fees charged by a third-party lab are not refunded by Pure Hormone Health. If we cannot legally care for you in your state, any membership charge taken in error will be refunded.

Failed Payments

If a monthly payment fails, we will contact you and may pause new shipments until the account is current. Prescriptions already in process at the pharmacy follow that pharmacy's policy.
